Title: Home HbA1c Testing Increases Frequency and Leads to HbA1c Reduction in Patients with Type 2 Diabetes: A Retrospective Analysis

Introduction: The COVID-19 pandemic has created challenges in accessing routine testing for chronic diseases like diabetes. This study investigates the effectiveness of a home hemoglobin A1c (HbA1c) test kit intervention in increasing HbA1c testing frequency and reducing HbA1c levels among patients with type 2 diabetes (T2D). Additionally, the study examines the impact of self-reinforcement education on the maintenance of HbA1c testing.

Objective: To evaluate whether a home HbA1c test kit intervention increases HbA1c testing rates and leads to changes in HbA1c levels 6 months post-testing. Furthermore, to assess whether self-reinforcement education improves the maintenance of HbA1c testing.

Study Design: Retrospective analysis of a randomized, controlled quality improvement intervention among members with T2D in a large commercial health plan.

Methods: The study included 41,214 fully insured members with T2D who had not undergone an HbA1c test in the past 6 months or had only one test in the last 12 months. Participants were randomly assigned to either a control group or an at-home HbA1c testing intervention group. The intervention group received either an opt-in or an opt-out HbA1c test kit shipment. A third group received self-reinforcement messaging to encourage regular testing. The main outcomes measured were HbA1c testing rates and HbA1c levels.

Results: The at-home HbA1c test kits increased testing rates by 4.9% compared to the control group. Members with an HbA1c level of at least 7% who completed at-home testing experienced a 0.38% reduction in HbA1c 6 months post-intervention. Members who received self-reinforcement messaging had a 0.37% reduction in HbA1c levels post-intervention.

Conclusions: This study demonstrates that at-home HbA1c testing is an effective intervention for increasing testing rates and reducing HbA1c levels in patients with T2D. The findings suggest that home testing can help overcome barriers to routine testing and improve diabetes management.
